NHS e-Referral Service (Choose and Book) explained

📖 6 min readNHS.uk sourcedUpdated April 2026
In plain English

The NHS e-Referral Service (formerly Choose and Book) lets you pick your specialist appointment online or by phone. You receive a letter with a unique booking reference and password. You must book within a set time or your referral may lapse.

What is the NHS e-Referral Service?

When your GP refers you to a specialist, they may use the NHS e-Referral Service (e-RS). This system lets you choose which hospital or clinic you attend for your appointment. You receive a letter with a unique reference number and password to book online or by phone at 0345 608 8888.

What the letter contains

  • Unique booking reference — a number you need to book your appointment
  • Password — used alongside the reference to access the booking system
  • Booking deadline — the date by which you must book or your referral may lapse
  • List of providers — the hospitals or clinics your GP has approved for your referral

What to do if there are no appointments

If no appointments are available at your chosen provider, you have two options: wait for slots to become available (checking back regularly via the online portal), or ask your GP to add additional providers to your referral so you have more choice.

Your RTT clock started when your GP made the referral — regardless of whether you have booked yet. If you are approaching 18 weeks without an appointment, read our guide on how to chase your referral.

Your right to choose

Under the NHS Constitution, you have the right to choose where you are referred for your first outpatient appointment. This includes NHS Foundation Trusts, NHS Trusts, and some independent sector providers that have a contract with the NHS. See our guide on RTT rights for more.
